Cholesterol is not a poison your body is trying to get rid of. It is a substance your body deliberately makes, because without it you would not have functioning cells. Understanding that is the difference between following rules about food and understanding why the rules exist.
What cholesterol actually is
Cholesterol is a waxy, fat-like substance found in every cell you have. It is a structural component of cell membranes, and it is the raw material your body uses to build vitamin D, bile acids for digesting fat, and steroid hormones including cortisol, oestrogen and testosterone.
Because it is that important, your body does not leave the supply to chance. Your liver manufactures cholesterol itself — it does not depend on you eating any. This is the single most useful fact on this page, and it explains a great deal of what follows, including why the advice about eggs changed.
Why it needs carriers, and what LDL and HDL really are
Cholesterol is a fat. Blood is mostly water. Fat does not dissolve in water, so cholesterol cannot simply float through your bloodstream — it has to be packaged and transported.
The packages are called lipoproteins: a fatty core wrapped in a protein shell that the watery blood will accept. And this is where the everyday language goes wrong.
LDL and HDL are not two kinds of cholesterol. They are two kinds of delivery vehicle carrying the same cargo. “LDL cholesterol” on a lab report means “the cholesterol currently riding in low-density lipoproteins.” The cholesterol molecule is identical either way. What differs is the direction of travel.

This is why “good” and “bad” cholesterol is a useful shorthand and a slightly misleading one. Nothing about the molecule is good or bad. What matters is how much is in circulation and which way it is going.
How a problem develops
Arteries are lined with a smooth, single-cell layer called the endothelium. When LDL particles are abundant in the blood, some pass into the artery wall beneath that lining and become trapped there. Once trapped, they can be chemically altered — oxidised — and the immune system responds to them as it would to an injury.
White blood cells arrive, take up the altered particles, and become foam cells. Over years, that accumulation of cells, cholesterol and cellular debris forms a plaque: a thickened, inflamed patch in the artery wall. This process is atherosclerosis, and it is slow, silent and typically well advanced before it produces any symptom at all.
Two things follow from that. The first is that plaque narrows the vessel and reduces blood flow. The second, and the more dangerous, is that a plaque can rupture — and a ruptured plaque triggers a clot, which is the immediate mechanism behind most heart attacks and many strokes. This is why a number on a lab report is treated seriously even though you feel entirely well. The number is a measure of a process, not of a symptom.
Why dietary cholesterol turned out to matter less than expected
For decades the advice was to avoid cholesterol in food — which put eggs, prawns and liver on the restricted list. The reasoning was intuitive: eat less cholesterol, carry less cholesterol.
It did not hold up, and the reason is the liver. Because your body manufactures its own supply, it also regulates it. In most people, eating more cholesterol causes the liver to make somewhat less, and the effect on blood levels is modest. The US Dietary Guidelines dropped the specific daily cholesterol limit for this reason.
What replaced it is more useful: for most people, saturated fat and trans fat in the diet raise LDL more than dietary cholesterol does, because they change how the liver handles LDL rather than simply adding cargo. Two honest caveats. Response varies between individuals — some people are considerably more sensitive to dietary cholesterol than others. And foods high in cholesterol are often high in saturated fat too, which is a separate reason to watch them.
Reading your own numbers
A standard lipid panel reports total cholesterol, LDL, HDL and triglycerides, usually after a fasting period. What counts as a desirable LDL is not one universal number — it depends on your overall cardiovascular risk, which includes blood pressure, smoking, diabetes, family history and age. Someone who has already had a heart attack is given a much lower target than someone with no risk factors at all.
That is genuinely a conversation for your doctor, and it is the main reason this site does not publish target numbers as though they applied to everyone.
What actually moves the numbers
Research consistently associates several things with a better lipid profile: replacing saturated fat with unsaturated fat rather than with refined carbohydrate; soluble fibre from oats, beans, barley and fruit, which binds bile acids in the gut and prompts the liver to draw on circulating cholesterol to replace them; regular physical activity, which tends to raise HDL; and not smoking, which damages the endothelium directly.
Note the word replacing. The evidence for swapping saturated fat for unsaturated fat is much stronger than the evidence for simply eating less fat overall — a distinction that took the field a long time to sort out.
